See Leon Kass's writings. Some physicians today have the idea that (a) patient autonomy is the highest (or only) value in med ethics, and (b) it requires them to do whatever patients will pay them to do. (Kass is one of the best critics of this view.)
Did you all read the news article Sal cited? It sounds like this surgeon was operating quite ethically, trying to correct some of the damage Jackson's face had suffered in his earlier operations.
